  • This form can also be found by going to www.registrar.calpoly.edu, then click “Forms”. Find “Former Student Evaluation Request”. Complete this immediately as the process can take 6-8 weeks.

Step 2: Email dsirois@calpoly.edu

  • State your catalog year (Example: 2009-11)
  • State the courses you believe you have outstanding

If you have been away from Cal Poly for more than 10 years

Step 2: Email dsirois@calpoly.edu

  • State your catalog year (Example: 2009-11)
  • State the courses you believe you have outstanding

Step 3: If you believe you have less than 16 units to graduate, you will need to request to graduate on a catalog older than 10 years. To begin this process, please submit the following to Dawn Sirois:

  • Current resume showing you have been working in the field relating to your major
  • One page statement showcasing how you have remained current in the Engineering field since leaving Cal Poly. This is also your opportunity to provide the Department any information they should be aware of when making their decision.
    • The reason we require currency in the field is due to the fact that if your request to graduate on a catalog older than 10 years is approved you will eventually receive a degree in the current year.
  • 2 letters of recommendation
    • Current or former employers or colleagues that can verify your work experience
  • Give permission to the university to contact references
    • This can be done by emailing Dawn Sirois (Ex: I give Cal Poly faculty and staff permission to contact my references)

Once all these documents are submitted and the Office of the Registrar has confirmed your catalog and remaining degree requirements, your request will be routed to the Department Chair, the Dean, and the Vice Provost of Academic Programs for review. This process could take up to 4 weeks.

If the University approves your request to graduate on a catalog older than 10 years, Dawn Sirois will work with you on how to complete your remaining requirements.
