• Proposed Course of Study
• Official Copy of University Transcripts
• Letter of Recommendation from a Cal Poly Faculty Member
• Statement of Purpose for Participating in the College of Engineering Exchange Program
• Assumption of Risk and Release Agreement
• Medical Information and Authorization Form
• Student Participant Agreement
